UPCCC APPLICATION

 

Enrollment at the Upper Perkiomen Child Care Center and School Age Program is open year round to children from six weeks of age to thirteen years old. Enrollment shall be granted without regard to a child's race, color, creed, religion, national origin, gender, or disability; and without regard to a parent(s) or guardian(s) race, color, creed, religion, age, national origin, gender, pregnancy, political belief or disability.

Parents and/or legal guardians and families are encouraged to tour the center prior to enrollment of their child(ren).

Initial enrollment is contingent upon receipt of the completed enrollment forms and registration fee. This includes, but is not limited to, an agreement form, an application, an emergency contact form,  a child health assessment form and an updated immunization record.
